Job Description:

Job Title: Family Nurse Practitioner (FNP)

Location: Remote – Work from Anywhere in the U.S.

Duration: 6 Months + Possibility of Extension

Schedule: Every other weekend (PRN) with additional hours available based on business needs.


The Telehealth Nurse Practitioner delivers patient care services through a remote technology platform. You will work in collaboration with a dedicated team of professionals as you independently provide holistic, evidenced based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients ranging in age 18 months and above. Encounters are documented utilizing an electronic health record (EHR).


Duties:

  • Provide evidence-based primary and family care, including assessment, diagnosis, treatment, health counseling, and care planning.
  • Manage clinic operations such as opening/closing, inventory, insurance verification, payments, and maintaining a clean environment.
  • Handle patient appointments, follow-ups, and perform routine examinations.
  • Conduct diagnostic tests, educate patients on healthy lifestyle choices, and coordinate with healthcare professionals.
  • Promote patient involvement through health screenings, behavioral health support, wellness services, and chronic disease management.


Requirements:

  • Master's degree as a Family Nurse Practitioner (FNP).
  • Active BLS certification (AHA or ARC only).
  • Unrestricted RN and APRN licenses with no disciplinary actions.
  • High-speed internet (minimum 25 Mbps download / 3 Mbps upload).
  • Experience with Electronic Medical Records (EMR/EHR).


Experience:

  • Master’s degree from a Family Nurse Practitioner program with current National Board Certification and state license to practice as an Advanced Practice Nurse.
  • At least 1 year of recent Primary Care experience as a Nurse Practitioner/Physician Assistant preferred.
  • Knowledge of quality measurement and chronic disease management.
  • Self-motivated with strong problem-solving skills and patient advocacy.
  • Effective verbal, written, and electronic communication skills.
  • Strong organizational skills with the ability to multitask.
  • Demonstrates initiative, adaptability, and flexibility.
  • Ability to work remotely within a clinical care team and practice autonomously without direct supervision.
  • Proficient in information management and technology.
  • Willing to obtain multi-state licensure.
  • Able to collaborate frequently with healthcare professionals to ensure quality patient care.
  • Candidate questionnaire must be included on the resume.
  • Oversee the implementation of new care models or technologies to improve patient care and operational efficiency.
  • Ensure accurate documentation and reporting of case management activities and patient care outcomes.
