Seeking an experienced Licensed Practical Nurse (LPN) to provide comprehensive remote care management for patients, focusing on value-based care initiatives.

Seeking an experienced Licensed Practical Nurse (LPN) to provide comprehensive remote care management for patients, focusing on value-based care initiatives.
Key Responsibilities
- Manage patient caseload to close care gaps through a resolution-driven approach.
- Coordinate community resources and provide education on chronic condition management.
- Perform medication management, including reconciliation and adherence.
- Ensure timely delivery of services like Home Health, DME, and Home Infusion.
- Assist with referrals, appointment scheduling, and address patient questions.
- Active multi-state or compact LPN license.
- Minimum of 3 years of patient-facing experience.
- Technical proficiency in Microsoft Office.
- Excellent communication skills, both written and verbal.
- Experience with Epic and bilingual skills are a plus.
- Must have high-speed Wi-Fi and a HIPAA-compliant workspace.
- Position involves sedentary work, requiring long periods of sitting, clear communication, and attentive listening.
