Role Overview: A Licensed Practical Nurse (LPN) working remotely to deliver value-based care management for patients.
Responsibilities include: Managing patient census, closing care gaps, coordinating community resources, providing patient education on chronic conditions, managing medications, facilitating timely services such as home health and DME, resolving patient inquiries, and managing referrals and appointments.
Qualifications and Skills: Active multi-state or compact LPN license, technical proficiency in Microsoft Office, excellent communication skills, and preferred Epic experience and bilingual abilities.
Requirements: Must have high-speed Wi-Fi, a HIPAA-compliant workspace, and physical ability to sit for long periods and communicate clearly.
